Services

Home Health Services Available through Commonwealth Home Health Care, Inc. Skilled Nursing Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) work jointly with your doctor to provide home care services on a one-on-one basis. Their assessment, establishment of nursing care plans, skilled treatment and support provide the high quality care you need to recover right at home. A registered nurse is available for admissions, emergencies or questions/ concerns 24 hours a day.

Home care nurses are available to provide:

Skilled nursing assessments. Wound care and dressing changes. Injections Catheter/Ostomy care. Intravenous fluid/medication management. Diabetes. Mellitus Care. Vital Sign Monitoring. Educating patient and families concerning safety, medications, diet/nutrition and disease processes.

Certifed Home Health Aides

Certified Home Health Aids are nursing assistants who provide personal care such as:

Bathing/Dressing. Personal Hygiene. Ambulation Meals. Companionship. Light Housekeeping.

Physical Therapy

Our physical therapy team will come to your home to assess and treat functional impairments that result from injury, stroke, surgery, fractures or other medical conditions. Physical therapy services include; Therapeutic Exercises, Gait Training, Instruction on using wheelchair/walker/cane, Pain Management Techniques, Education on Safety Precautions.

Occupational Therapy

Our occupational therapist is a qualified healthcare profes- sional who directs, supervises, evaluates and provides occupational therapy services to patients in their homes by providing: Rehabilitative care for patients needing assistance. Training in activities of daily living. 

Speech Therapy

Speech Therapy helps patients to improve speech/communications. Evaluate and treat swallowing disorders 

Medical Social Services

Our MSW staff help patients and family members under- stand and cope with problems caused by illness/injury through: Assessment of emotional and social factors related to patient and family. Evaluate the home and family environment. Counseling and education to maximize strengths and coping skills. Provide guidance with problem solving interventions. Assistance in financial matters.

Coordination of community resources.

We are a Full Range of services with MD referral!

Laboratory Tests. Infusion Medications and Inhalation Supplies.

What Types of Conditions Indicate A Need for Home Health Care?

What Are The Benefits 0f Home Health Care?

Care is provided in the privacy and comfort of your own home. All services are coordinated with your physician(s). 24-hour access to a skilled nurse. Home care may prevent hospitalization/ re-hospitalization. Home care enables patients to leave the hospital setting sooner Home care allows for shorter recovery times.

Your doctor, your family/friends. You can also refer yourself.

Insurance companies, nursing homes, hospitals and rehabilitation centers may also refer you.

What Will My Insurance Cover? 

Your insurance may provide either complete or partial payment. Our professional team will explain your coverage and billy our insurance directly, hassle free. We accept: Medicare only.
